This form is for those students who wants to change the address and correction in name. Why we need to submit this form:
Some of students made error in filing forms with correct data or while typing data operator may make mistake but you should take immediate action to correct your database related to your admission form.
and For correction in the spelling of name please attach an attested photocopy of 10th class Certificate.
